Why Your Breath Is a Direct Dial on Your Nervous System
Your breath is one of the only autonomic functions you can consciously control — and that makes it a uniquely powerful lever. Slow, deliberate breathing at around six breaths per minute consistently activates the parasympathetic nervous system (the "rest and digest" branch), raises heart rate variability, and reduces circulating stress markers. This is not wellness mythology: it is measurable, reproducible physiology. The challenge is that the breathwork world is full of exaggerated claims layered on top of real but more modest effects. This guide cuts through both.

How Breathing Affects the Nervous System
Breathing is unique because it sits at the intersection of voluntary and involuntary control. When you breathe slowly and with an extended exhale, you stimulate the vagus nerve — the main highway of the parasympathetic nervous system — which sends a "calm down" signal throughout the body. Heart rate slows, blood pressure drops slightly, and cortisol activity decreases.
The key mechanism is baroreflex sensitivity and respiratory sinus arrhythmia: as you inhale, heart rate rises slightly; as you exhale, it falls. Slow breathing — particularly at around 6 breaths per minute — amplifies this oscillation and substantially increases heart rate variability (HRV). Higher HRV in this context indicates greater parasympathetic (vagal) tone, which is associated with better stress resilience and recovery. This is why breathwork is genuinely useful for HRV monitoring and why it appears in every serious stress management toolkit.
The Three Techniques Most Supported by Evidence
1. Slow Resonance Breathing (5–6 Breaths Per Minute)
This is the bedrock technique with the most consistent evidence base. Inhale for 5 seconds through the nose, exhale for 5 seconds through the nose or mouth. That gives you 6 breaths per minute — the "resonance frequency" at which the cardiovascular and respiratory systems couple most efficiently.
A systematic review of slow-breathing studies found statistically significant reductions in self-reported stress, anxiety, and resting blood pressure after several weeks of daily practice. Effects were modest in absolute terms but clinically meaningful for everyday stress management. The NHS recommends a version of this in its guidance on breathing exercises for stress.
How to use it: 5–10 minutes each morning, or during a stressful moment at work. Use a timer or a free app to pace your breath rather than counting in your head.
2. The Physiological Sigh
Researchers at Stanford identified the physiological sigh as the fastest way to reduce acute physiological stress in a 2023 randomised controlled trial. The technique: take a full inhale through the nose, then add a second short "top-up" inhale to fully inflate the alveoli, then exhale slowly and completely through the mouth. One to five repetitions is enough.
In the trial, participants who practised cyclic sighing for five minutes reported lower anxiety and improved mood compared to mindfulness meditation and cyclic hyperventilation. The mechanism is partly mechanical: the long exhale maximally engages the parasympathetic brake, and reinflating the alveoli clears CO₂ rapidly.
How to use it: In the moment — before a presentation, after a stressful call, when you feel overwhelmed. This is your emergency tool.
3. Box Breathing (4-4-4-4)
Inhale for 4 seconds, hold for 4 seconds, exhale for 4 seconds, hold for 4 seconds. Repeat for 4–8 cycles. Used widely in military and emergency services as a stress-regulation tool, box breathing works by slowing respiratory rate into the slow-breathing zone and providing a structured cognitive anchor that interrupts the physiological stress spiral.
The evidence is largely observational and mechanism-based rather than large RCT, but it aligns with what we know about slow breathing and is practically zero-risk for most people.
How to use it: Pre-performance, post-training cool-down, or as a transition ritual between work and home to stop you carrying stress into the evening.
What About 4-7-8 Breathing?
The 4-7-8 technique — inhale 4 seconds, hold 7, exhale 8 — enforces a very slow breathing rate and a long, extended exhale. The extended exhale is the key active ingredient (it drives parasympathetic activation), and the breath-hold adds a CO₂ tolerance element. It is popular for pre-sleep use and anecdotally effective for many people.
The hold of 7 seconds is modest enough to be safe for most healthy adults. However, if you have any cardiac condition, epilepsy, or are pregnant, check with your GP before using any breath-hold protocol. Never practise breath-holding alone in or near water — ever.
What Breathwork Does NOT Do
Some practitioners claim breathwork can "detoxify" the body, cure chronic conditions, or replace medical treatment. These claims are not supported by evidence. Breathwork:
- Does not cure anxiety disorders or depression (these need clinical treatment — signpost: NHS mental health services, Mind, your GP)
- Does not measurably "detox" the body (your liver and kidneys handle that)
- Does not produce the dramatic physical transformations claimed by extreme protocols in popular culture
- Has limited robust evidence for effects on athletic performance, immune function, or longevity when separated from the well-evidenced stress/HRV mechanisms
Hyperventilation-based protocols (such as Wim Hof-style breathing cycles) can cause light-headedness and, at extreme levels, loss of consciousness due to CO₂ washout. These must never be practised in or near water, in a bath, swimming, driving, or anywhere a sudden faint would be dangerous. This is a genuine safety issue, not a minor caveat.
Breathwork, Sleep and Recovery
Slow breathing before bed is a practical pre-sleep tool. The physiological pathway is clear: by lowering heart rate, blood pressure and sympathetic arousal in the 5–10 minutes before sleep, you reduce the time it takes to transition from wakefulness to sleep. This is particularly useful if you struggle to "switch off" after a stimulating evening.

If insomnia is persistent — more than three nights a week for over three months — this is chronic insomnia and warrants a GP conversation. Cognitive behavioural therapy for insomnia (CBT-I) is the NICE first-line treatment, not breathing techniques alone.
