Glutamine is a conditionally essential amino acid — one of 20 protein building blocks, and the most abundant free amino acid in both muscle tissue and blood plasma. Under normal healthy conditions, your body synthesises all the glutamine it needs from other amino acids. Only in extreme physiological stress — major surgery, severe burns, critical illness — does demand outpace supply, making it "conditionally" essential.

Despite this, glutamine has been a fixture in gym supplement aisles for decades, marketed for muscle building, immune defence and gut health. It is one of the UK's most widely purchased sports powders. The gap between that marketing and the actual evidence is striking.

Hypertrophy and muscle performance in healthy athletes — Grade D

This is the primary reason most lifters buy glutamine, and the evidence is decisively against it. Controlled trials in resistance-trained adults show no significant advantage for muscle mass, strength or power output when glutamine is added on top of adequate protein intake. Immune function in healthy athletes — Grade D

The immune angle was a popular hypothesis in the 1990s: intense training transiently suppresses circulating glutamine, therefore supplementing it should restore immune competence. Controlled studies in healthy athletes did not confirm this. Blood glutamine does dip post-exercise, but this dip is short-lived and does not translate into measurable immune impairment or illness risk. Gut health in clinical populations — Grade C

Here the picture is slightly more nuanced. In hospitalised patients — particularly those with gut permeability issues, inflammatory bowel conditions, or undergoing chemotherapy — intravenous or oral glutamine has shown modest benefit in some controlled trials. Intestinal cells (enterocytes) use glutamine as a primary fuel source, and in critical illness, supply may genuinely be insufficient. Grade C evidence: limited and mixed, but not zero. It is a single, well-characterised amino acid. That said, the honest advice here is: do not buy it at all if muscle, performance or immune function is the goal. The money is better spent on food.
