What calcium actually does
Calcium is not just structural. It drives muscle contraction (including the heart), nerve transmission, blood clotting, and enzyme function. Your body defends blood calcium levels fiercely — if dietary supply drops, it mobilises calcium from bone. Over time, that draws down bone mineral density and raises fracture risk. Getting enough in the diet is therefore less about a supplement "working" and more about stopping a slow, silent drain on skeletal capital.
Does it work? The evidence
Bone health when intake is low — Grade B. The NHS and NIH Office of Dietary Supplements both recognise adequate calcium intake as essential for peak bone mass in youth and for slowing bone loss in later life. Meta-analyses of calcium supplementation trials show modest but consistent improvements in bone mineral density when baseline intake is low. The effect is real but moderate — supplements are no substitute for a calcium-rich diet, weight-bearing exercise, and adequate vitamin D (which is required for calcium absorption in the gut).
Fracture prevention from supplements alone — Grade C. The picture is less clear when it comes to fractures. A 2012 USPSTF review and subsequent analyses found that calcium plus vitamin D supplementation in community-dwelling postmenopausal women produced inconsistent fracture reductions. How to take it
The UK RNI is 700 mg per day. The first step is not to reach for a pill — it is to audit your diet. A 200 ml glass of semi-skimmed milk delivers roughly 240 mg; a pot of plain yoghurt around 200 mg; a 100 g serving of tinned sardines with bones around 380 mg; a 100 g portion of firm tofu (calcium-set) around 350 mg; 200 ml of a fortified oat or soy milk around 240 mg. Run the numbers before supplementing.
If you genuinely cannot close the gap through food:
- Dose: Supplement only the shortfall, not the full RNI. If you eat roughly 400 mg from food, a 300 mg supplement is appropriate — not 1000 mg.
- Maximum per dose: ≤500 mg at a time. Absorption saturates above this; splitting doses across meals is more effective.
- Form matters: Calcium carbonate (the most common, cheapest) requires stomach acid — take it with food. Calcium citrate is gentler on the stomach and can be taken without food; it is the better choice if you use a proton-pump inhibitor (PPI) such as omeprazole.

Iron and zinc competition. Calcium competes with iron and zinc at intestinal absorption sites. If you are taking iron tablets — which should always be blood-test-guided — separate them from calcium by at least two hours. The same logic applies if you are taking zinc.
Medication timing. Calcium can reduce the absorption of levothyroxine (thyroid hormone), certain antibiotics (tetracyclines, fluoroquinolones), and bisphosphonates (alendronate — used for osteoporosis). Space these medications by at least two hours either side of a calcium supplement.
How to buy it well
Calcium is not an arena riddled with contamination risk in the way botanicals or fish oils can be, but a few principles apply:
- Dose transparency. Check the elemental calcium content, not the salt weight. Calcium carbonate is 40% elemental calcium; calcium citrate is 21%. A label claiming '1000 mg calcium carbonate' provides ~400 mg elemental calcium.
- Third-party testing. Look for an Informed-Sport stamp or equivalent if you are a tested athlete — calcium supplements sometimes contain trace contaminants from natural mineral sources. For most people, a reputable UK pharmacy brand is sufficient.
- Avoid mega-doses. Products selling 1200–1500 mg per tablet push well beyond what most people need to supplement and the doses where the CV risk signal has been observed. Buy small, supplement precisely.
- Stack sensibly. Calcium + vitamin D combinations are well-studied and convenient if you need both. Check vitamin D status first so you can match the D dose to your actual need rather than accepting a one-size-fits-all tablet.

Calcium is an essential mineral, not a high-performance supplement. If your diet reliably delivers 700 mg per day — and for most UK adults who eat any dairy or fortified foods, it likely does — a supplement adds no proven benefit and a debated safety question. If your diet genuinely falls short, a targeted, appropriately dosed supplement (≤500 mg per dose, with vitamin D, from food where possible) is a smart, evidence-backed move.
