The honest picture on testosterone and ageing in men
Testosterone does decline with age in men — this is well-established. The most important thing to understand: the four biggest drivers of the symptoms associated with 'low T' — fatigue, reduced libido, increased body fat, low mood, loss of muscle — are sleep deprivation, physical inactivity, visceral obesity and depression. All four are powerfully modifiable. Lifestyle intervention is the correct first line of action. A GP blood test is the correct diagnostic step if symptoms are significant. What actually happens to men's bodies with age
Beyond testosterone, male physiology undergoes several significant shifts from the mid-30s onward:
Muscle and strength. Muscle mass declines approximately 3–8% per decade after age 30, accelerating after 60 — a process called sarcopenia. Strength and power decline even faster than mass. This creates a negative feedback loop: more fat → lower testosterone → more fat. Breaking it requires both nutrition and structured training.
Sleep architecture. Deep slow-wave sleep — the most physically restorative stage — declines with age in men. Growth hormone is predominantly released during slow-wave sleep. Disrupted or shortened sleep is directly associated with lower testosterone, higher cortisol, impaired muscle recovery and increased appetite for high-calorie foods. Bone density. Men lose bone mass with age too, though typically later and less acutely than women. By their 50s and 60s, bone density concerns become relevant for men as well. 'Andropause' vs clinical hypogonadism: the key distinction
The NHS explicitly notes that 'andropause' or 'male menopause' is a contested term. Female menopause involves a rapid, complete cessation of ovarian hormone production — an unmistakable biological event. In men, the testosterone decline is gradual, partial and highly variable between individuals. Many men maintain clinically normal testosterone levels into their 70s and beyond.
Late-onset hypogonadism is the clinical term for men who have consistently low testosterone (typically below 10–12 nmol/L on morning fasting tests, repeated on two occasions) combined with specific symptoms including sexual dysfunction, loss of morning erections, reduced muscle mass, and unexplained fatigue. This is a diagnosable medical condition, distinct from the background gradual decline experienced by all men.
The crucial distinction: feeling tired, putting on weight, losing motivation and noticing reduced gym performance in your 40s does not mean you have hypogonadism. These symptoms are far more commonly explained by overwork, poor sleep, inadequate exercise and lifestyle drift. A GP blood test is the only way to tell the difference.
What the lifestyle evidence actually shows
For men in the normal age-related decline band — not clinical hypogonadism — the evidence for lifestyle interventions is strong and consistent:
Resistance training
Regular resistance training is the most powerful modifiable lever for maintaining testosterone, muscle mass, strength and metabolic health in ageing men. Large compound movements produce the strongest hormonal response. The WHO and NHS recommend muscle-strengthening activity at least 2 days per week; for men prioritising body composition and vitality, 3–4 structured sessions is the evidence-supported sweet spot.

A study in young men (published in JAMA, 2011) found that restricting sleep to 5 hours per night for one week reduced daytime testosterone levels by 10–15%. Chronic partial sleep deprivation — which is the default state for many busy men — is quietly suppressing testosterone, elevating cortisol, degrading recovery, and fuelling fat gain. Even moderate-but-regular intake (above 14 units per week — the NHS low-risk guideline) can compound the age-related decline. Stress and chronically elevated cortisol also suppress testicular function over time. These are modifiable factors that often go unaddressed while men focus on supplements.

Seek a GP appointment if you experience:

A GP will arrange morning fasting blood tests (total testosterone, LH, FSH, SHBG, prolactin, thyroid function and a metabolic panel). Two separate morning tests are needed before any diagnosis. The Endocrine Society guidelines and NHS guidance are clear: TRT should be prescribed only after confirmed diagnosis, not as a lifestyle intervention or optimisation tool.
TRT: what you need to know
Testosterone Replacement Therapy is a legitimate, effective treatment for men with confirmed clinical hypogonadism. It is not a general anti-ageing intervention, a performance enhancement tool, or a shortcut around lifestyle. The risks of TRT — which should be discussed fully with a clinician — include:
- Suppression of natural production: exogenous testosterone signals the hypothalamic-pituitary axis to stop producing LH/FSH, leading to testicular atrophy and infertility
- Polycythaemia (elevated red blood cell count): raises cardiovascular risk if not monitored
